Job summary

Integrated Services for Behavioral Health (ISBH) in Ohio is seeking a Care Coordinator to provide service linkage and coordination for children, youth, and families with behavioral health needs. The role involves engaging families, coordinating home, school, and community services, and building community partnerships.

Minimum requirements include experience with behavioral health services for families, knowledge of wraparound, and strong communication.

Qualifications

• Experience serving children and families in behavioral health or related public services.
• Hi-Fidelity Wraparound knowledge preferred (Certification provided).
• Two years in a coordinated care coordination role preferred.
• Experience with autism spectrum disorders and developmental disabilities preferred.
• Strong organizational skills and ability to manage multiple tasks.
• Excellent verbal and written communication; able to operate in an Internet-based office.

Responsibilities

• Identify care coordination needs with families and align with service delivery standards.
• Engage families to define cultural factors influencing care planning and engagement.
• Identify strengths of children, youth, and families for care coordination.
• Coordinate family-based services at home, school, and community.
• Ensure services on care plans are appropriate, least restrictive, and meet safety needs.
• Engage and build relationships with families to support integrated care plans.
• Develop partnerships with community resources to meet diverse needs.
• Maintain documentation, participate in program evaluation, and attend planning meetings.
• Remain current with training requirements including High Fidelity Wraparound.
