Job summary

Integrated Services for Behavioral Health is seeking a Care Coordinator to engage with children, youth, and families across home, school, and community settings in Southeast and Central Ohio.

You will link services, coordinate care, and support team-based planning while upholding cultural humility and collaboration across systems. Competitive hourly pay with a $3,000 hiring bonus and strong benefits are offered.

Qualifications

• Experience providing services to children and families in behavioral health or related public sector fields.
• Knowledge of Hi-Fidelity Wraparound preferred; certification provided on employment.
• Strong understanding of family systems and community resources.

Responsibilities

• Identify care coordination needs with families to ensure best outcomes for children, youth, and families.
• Engage families to define cultural factors influencing strengths and family interactions for care planning.
• Coordinate family-based services in home, school, and community settings.
• Ensure services on care plans are appropriate, least restrictive, and meet safety and treatment needs.
• Build positive relationships with children, youth, and families with child and family teams.
• Develop collaborations with community resources to meet diverse needs.
• Maintain documentation, participate in program evaluation, attend meetings and training.
