Job summary

Crossroads Treatment Centers seeks a Care Coordinator to support patients on their recovery path, meeting with patients, coordinating services, and documenting activities in records. You will work with local referral agencies to provide needed services.

You will conduct monthly care coordination meetings, collaborate with buprenorphine‑waivered prescribers, and perform duties assigned by the Operations Director of OBOT Services. This role offers meaningful frontline impact.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Psychiatric Rehabilitation, Sociology, Counseling, Vocational Rehabilitation, or Human Services Counseling and at least one year of experience with substance use disorders.

Responsibilities

• Meet face-to-face with patients to facilitate recovery services and document in the patient’s record.

Education

Bachelor’s degree in Social Work or related Human Services field

RN licensure with experience serving individuals with SUD

CSAC or CSAC‑Assistant under supervision
