Job summary

Memorial Healthcare is seeking a community program coordinator to provide case management, care coordination, and informal counseling for youth and families enrolled in the program. You will evaluate client needs, develop daily activity schedules, arrange transportation to appointments, and report outcomes to the Team Leader, while maintaining a flexible, high-communication approach in a busy environment.

Requires strong accountability, accuracy, and the ability to work with minimal supervision;

Qualifications

• Bachelor's degree required.
• Must have valid Florida driver's license and reliable personal vehicle.
• Basic Life Saving (BLS) certification within six months of hire.

Summary

Provides support to community youth and their families including informal counseling, care coordination, and overseeing the daily operations of the program’s services.

Responsibilities

Provides case management, care coordination, and informal counseling for clients and families enrolled in program.Evaluates and assesses the needs of the clients and families.Develops and maintains daily activity schedules.Transports clients and families to appointments and/or community resources, when needed.Per grant requirements, completes surveys and/or assessments with clients and families and reports outcome data to program’s Team Leader.
