Memorial Healthcare System is seeking a dedicated program coordinator to support community youth and families through case management and care coordination. The role involves informal counseling, evaluation of client needs, and daily activity planning to ensure service delivery aligns with grant requirements.

The ideal candidate will handle transportation, data reporting, and collaboration with the program team while maintaining flexibility to meet client needs and travel across Broward County.

Qualifications

• Requires Bachelor’s degree.
• Must have valid Florida driver’s license and be cleared for Memorial Insurance.
• Ability to work with minimal supervision in a high-stress environment.
• Flexibility to work a varied schedule.
• No prior work experience required.

Summary

Provides support to community youth and their families including informal counseling, care coordination, and overseeing the daily operations of the program's services.

Responsibilities

Provides case management, care coordination, and informal counseling for clients and families enrolled in program.
Evaluates and assesses the needs of the clients and families.
Develops and maintains daily activity schedules.
Transports clients and families to appointments and/or community resources, when needed.
Per grant requirements, completes surveys and/or assessments with clients and families and reports outcome data to program's Team Leader.
