You will conduct in-home assessments, develop individualized care plans, and coordinate with the broader care team to ensure safety and quality. You will build strong relationships with clients, families, and referral partners while maintaining thorough documentation and ensuring adherence to policies and regulations.

Qualifications

• 1 year of experience in care coordination or related role.
• Experience in home care or community-based healthcare is preferred.
• Strong communication and organizational skills.

Responsibilities

• Conduct in-home client assessments and supervisory visits.
• Develop, implement, and update individualized care plans.
• Build relationships with clients, families, caregivers, and referral partners.
• Listen to clients and families, understand concerns, and address solutions.
• Support caregiver onboarding, training, and supervision.
• Monitor quality and address concerns before they escalate.
• Ensure compliance with policies, Arizona regulations, and insurance requirements.
• Maintain accurate, timely documentation.
• Collaborate with Scheduling, Recruiting, HR, and Operations.
• Identify opportunities to improve client and caregiver experience.
