Qualifications

• BA/BS in human services required.
• At least 2 years in human services with mental illness/substance use/disabilities.
• 1 year in-person case management preferred.
• Ability to function as part of an interdisciplinary team.
• Valid driver’s license and personal vehicle for business use.

Responsibilities

• Coordinate and monitor physical and behavioral health needs for adults with severe mental illness.
• Complete assessments and develop person-centered care plans.
• Coordinate referrals and link to community services.
• Collaborate with community agencies and healthcare providers.
• Provide in-person home and community-based care with transportation.
