Job summary

Saint Regis Mohawk Tribe Health Services seeks an RN/Care Coordinator to support mental health therapists, psychiatrists, and the outpatient ACDP staff. You will triage referrals, provide injections, assist with case management, and link clients to medical and behavioral health services.

Strong communication and computer skills are essential. Licensed in NY with at least one year of medical-surgical RN experience; behavioral health experience preferred.

Qualifications

• Must hold a New York State Registered Nurse license.
• Minimum 1 year medical-surgical RN experience.
• Preferred 1 year experience in behavioral health, inpatient or outpatient.
• Excellent oral and written communication and computer skills.

Responsibilities

• Exhibit a friendly and courteous manner when dealing with clients and colleagues.
• Work with behavioral health team to carry out treatment plans.
• Coordinate service delivery with other agencies and programs within Health Services.
• Triages referrals and assesses emergent needs for treatment.
• Provide injections to clients receiving injectable psychotropic medications.
• Assist therapists with care coordination and case management.
• Link clients to physical health services and supports as needed.
• Monitor client health and medication management as per treatment plans.
• Facilitate wraparound and care coordination meetings when needed.
• Engage in outreach and screenings at community sites and events.
