Mental Health Association - Western MA invites applications for a Care Coordinator within the GRIT program. You will connect clients to behavioral health resources, coordinate care including MAT and psychiatric services, and help develop aftercare and discharge plans.
This role emphasizes trauma‑informed, person-centered, culturally responsive support. Required: high school diploma or GED; relevant experience in human services; MAP, CPR and first aid certifications; valid driver’s license.
The Care Coordinator supports individuals by providing information, developing connections with community and behavioral health resources, and coordinating care services, including Medication-Assisted Treatment (MAT) and psychiatric care. The role also involves coordinating referrals and establishing aftercare and discharge plans.
Pay Rate: $19 - $21 an hour
Open Shifts:
- Monday through Friday 8am-4pm (40hrs)
- Monday through Friday 9am-5pm (40hrs) two open positions
- Monday through Friday 10am-6pm (40hrs)
The GRIT program at MHA provides residential rehabilitation for individuals with substance use challenges and co‑occurring mental health conditions. Residents participate in individualized counseling, recovery‑focused programming, and community activities such as education, job training, or employment, all within a structured and supportive environment. With locations in Springfield (men’s programs) and Holyoke (LGBTQIA+ program), GRIT combines the comforts of home with access to vital resources. A skilled, multidisciplinary team works closely with each resident to support recovery, personal growth, and long‑term stability, while creating personalized aftercare plans for a successful return to the community.
