Job summary

New Horizons In-Home Care is seeking a coordinated scheduling and assessment professional to manage timely client assessments, develop care plans, and ensure ongoing client evaluations. The role emphasizes compassionate, accountable service and outstanding client satisfaction while aligning with the agency’s culture and values.

The position involves collaboration with external case managers, RN oversight for nursing clients, and on-going coordination to deliver high-quality home care services.

Qualifications

• Minimum of one year customer service experience in a healthcare related field.

• Strong written, verbal, and presentation skills with diverse populations.
• Ability to multi-task and stay calm in a fast-paced environment while paying attention to detail.

Responsibilities

• Perform initial assessments for Clients with external Case Managers (Medicaid, VA, ODDS, etc.).
• Perform Quarterly Assessments for all non-nursing clients.
• Create new Client Care Plans and Task Lists in coordination with the RN for nursing services clients.
• Evaluate and report quality of client-site services.
• Identify client physical, nutritional and hygiene supports including transfer and mobility.
• Develop home safety plans.
• Participate in on-call rotation, provide phone support and caregiving assignments.
• Maintain confidentiality of client, employee, and agency operations.

This position is responsible for the timely scheduling, coordination and recording of new client assessments and the development of client care plans; reporting, and providing on-going existing client assessments.
