Job summary

Rocky Boy Health Center is seeking a Care Coordinator to provide outreach, care coordination, health education, and disease prevention for patients in the Rocky Boy and Box Elder communities. The role collaborates with healthcare providers and community resources to help patients access services and follow care plans.

Responsibilities include coordinating referrals and transportation, conducting outreach, educating patients on preventive care, and maintaining outreach records.

Qualifications

• High school diploma or equivalent.
• Current CPR and First Aid certification.
• Ability to read, write, and communicate effectively in English.
• Knowledge of the Rocky Boy Reservation, its communities, and local health needs.
• Ability to work independently and collaboratively with healthcare professionals.
• Current valid Montana driver’s license.
