Job summary

Agency for Community Treatment Services, Inc. (ACTS) is seeking a community-based treatment coordinator for our Addictions Receiving Facility in Tampa, FL. You will help coordinate admissions across programs and motivate clients to engage in treatment.

Role requires a Bachelor’s in a human services field, CPR/First Aid, and clearance under Florida Level 2 background screening, plus strong communication and customer service skills. On-site with daily travel to other ACTS locations.

Qualifications

  • Bachelor's Degree in a human services related field.
  • Ability to participate in Welle (crisis intervention) and CPR/First Aid training.
  • Eligibility status as determined by Level 2 background screening per Florida Statutes, Chapter 435 and Local Law.
  • Ability to lift, push, pull, carry, reach, stoop and bend.
  • Full participation in transportation-related training activities, including on-the-road driving.
  • Excellent verbal and written communication skills.
  • Excellent interpersonal and customer service skills.

Responsibilities

  • Coordinates admissions between ACTS addiction receiving, outpatient or residential services and other community-based programs.
  • Engages and motivates clients to enter or re-enter treatment settings to accomplish treatment plan goals.
  • Ensure smooth transitions to the next level of care (warm hand-offs) to avoid gaps in service and respond rapidly for needed support.
  • Facilitate connection with peer support specialists, education, employment, housing and other wraparound services.
  • transports clients and/or materials to medical, legal, recreational and other community appointments supporting recovery.
  • Attend multidisciplinary team meetings to review case status.
  • Attend court hearings, as needed.
  • Prepare documents including service delivery reports.
  • Prepares data reports related to placement success outcomes.
  • Other duties as assigned.

What You'll be Doing:
