Job summary

The Mental Health Association of Columbia-Greene Counties Inc. is currently hiring three full-time Care Coordinators to support Medicaid-eligible individuals across Columbia and Greene counties.

Care Coordinators coordinate behavioral and physical health care, working with Hudson River Health Care as Health Home provider. The role requires 75% fieldwork, including home visits and community assessments, to develop personalized wellness plans and link clients to essential services for improved

Qualifications

  • Bachelor's degree in a social service or counseling discipline OR Registered Nurse with bachelor’s degree and at least 2 years’ experience in providing direct services in the mental health field or in linking clients to a broad range of services in the community.
  • Experience in providing direct service in the mental health or social service field and linking clients to services in the community is necessary.
  • Candidates must have reliable transportation to travel to client locations and other necessary work sites.
  • Proficiency in, or ability to learn, online technology systems is critical.
  • Ability to work with a diverse population of urban, suburban and rural cultures and with community members from varying socioeconomic backgrounds.
  • Demonstrate a commitment to Diversity, Equity, and Inclusion through continuous development, modeling inclusive behaviors, and proactively managing bias.
  • Must be able to work regularly outside the office and in the field to engage with clients.
  • Excellent oral and written communication skills.
  • Familiarity with Greene County and knowledge of managed care is a plus.
  • Regular and steady attendance is a requirement for all positions.

Responsibilities

  • Connect and refer clients to appropriate services that support their care plan and treatment goals, including medical/behavioral health care, patient education, and self-help recovery and self-management.
  • Conduct case reviews with interdisciplinary team to monitor/evaluate client status/needs.
  • Advocate for services and assist with scheduling of needed services.
  • Collaborate with treating clinicians to ensure services are delivered and that changes in medical treatment or conditions are addressed.
  • Monitor and support clients in making scheduledmedical/behavioral/social service appointments.
  • Provide crisis intervention when needed.Revise care plans and goals as necessary.

The Mental Health Association of Columbia-Greene Counties Inc. is currently hiring for three full-time positions of Care Coordinator.

Care Coordination offers services across both Columbia and Greene counties. Our Care Coordinators assist Medicaid-eligible individuals in coordinating both behavioral and/or physical health care, regardless of diagnosis. They work with those who qualify to ensure they access the best possible services for physical and mental health. These services are provided through our partnership with Hudson River Health Care, our Health Home provider. Care Coordinators collaborate with other providers to develop personalized wellness plans, focusing on rehabilitation, recovery, and linking individuals to essential clinical services for improved daily health management.

Position Type:

Full Time-40 Hours/Non-Exempt

Pay Rate:

$22.00 per hour with an increase to $24.00 per hour after one year of employment

Work Location:

Greene County, NY

Shift Schedule:

Monday-Friday 8:30am-4:30pm

Job duties include:

Requirements
  • Must have a bachelor's degree in a social service or counseling discipline such as social work, psychology, nursing, rehabilitation, community mental health social work, psychology, or related field OR Registered Nurse with bachelor’s degree and at least 2 years’ experience in providing direct services in the mental health field or in linking clients to a broad range of services in the community.
  • Experience in providing direct service in the mental health or social service field and linking clients to services in the community is necessary.
  • Candidates must have reliable transportation to travel to client locations and other necessary work sites.

All offers are contingent upon successful Statewide Central Registry (SCR), fingerprint clearance and possession of a valid NYS driver’s license with a MVR eligible for agency insurance coverage.
