Job summary

VillageHealth seeks a dedicated Patient Care Coordinator to support Chronic Kidney Disease patients. Coordinate with providers and VillageHealth nurses to ensure ongoing care and act as a single point of contact for CKD patient resources.

You will perform outbound and inbound calls, schedule appointments, assist with referrals, and maintain confidential information with discretion. MS Office skills and strong communication are essential.

Qualifications

  • High school diploma or equivalent.
  • At least two years of experience in a professional office environment.
  • Intermediate proficiency in MS Word, Excel, PowerPoint, and Outlook.
  • Excellent verbal and written communication skills.
  • Ability to handle confidential information with discretion and judgment.
  • Pre-employment substance abuse testing and background verification checks.

Responsibilities

  • Coordinate with providers and VillageHealth Nurses to support CKD patients.
  • Serve as a single point of contact to coordinate resources for VillageHealth CKD patients.
  • Make outbound calls to patients to understand clinical needs and connect them with resources.
  • Make outbound calls to providers to arrange patient appointments or follow-ups.
  • Answer inbound calls from patients, providers and Village resources.
  • Follow up with patients to ensure needs are met and schedule future check-ins.
  • Coordinate with clinical resources and providers to ensure continuum of care.
  • Monitor patient hospitalizations and follow up with a VillageHealth Nurse.
  • Notify patients of locations and appointment times.
  • Provide patient education materials and send letters to physicians for enrollments and appointments.
  • Assist with the referral process.
  • Perform other duties as assigned.

  • Follow up with patients to ensure needs are met and schedule future check-ins
  • Coordinate with clinical resources and providers to ensure a smooth continuum of care
  • Monitor patient hospitalizations and follow up with a VillageHealth Nurse
  • Notify patients of locations and appointment times
  • Provide patient education materials and send letters to primary care physicians, nephrologists, and specialists for new enrollments and appointments
  • Assist with the referral process
  • Perform other duties as assigned
Requirements

  • Excellent verbal and written communication skills
