The Care Coordinator serves as the primary point of contact for care coordination needs of individuals seeking services at the ELC. The Care Coordinator assists individuals in navigating access to whole-person care (behavioral and physical), facilitates screening and assessment tools, provides referral and coordination for internal and external health needs, and helps to engage individuals in recommended care. The Care Coordinator works directly with individuals experiencing behavioral health challenges, as well as with individuals’ supports, internal ELC team members and external community agency partners. This position reports to the Quality Assurance Director or their designee.
