Job summary

Outreach Health in Amarillo, TX, is seeking a Care Coordinator to interact with clients and caregivers, optimize scheduling, and support safe, high-quality care. You will build strong caregiver relationships and help coordinate care with Care Managers to ensure timely, accurate service delivery.

The role requires strong organizational, communication, and problem-solving abilities, plus proficiency with MS Office and EHR systems.

Responsibilities

  • Maintain quality service by following Outreach SOPs to onboard new clients timely and ensure accuracy.
  • Communicate effectively with referral sources to correct client registration or authorization issues
  • Maintain clients’ EHR by entering new information timely and accurately
  • Address client concerns timely and with empathy
  • Support other Care Team members to provide the best customer service and foster trust
  • Recruit and onboard caregivers, including interviews, paperwork, reference checks, and training coordination
  • Coordinate with case managers and practitioners to ensure continuity of care
  • Assist in quality control of service delivery
  • Monitor and manage caregiver master schedule(s)
  • Ensure accurate and timely paperwork processing
  • Handle incoming/outgoing calls with kindness and professionalism
  • Monitor EVV codes and ensure accurate client visits and payroll submission
  • Generate timesheets and reconcile billing
  • Reconcile A/R and A/P as needed
  • Generate reports for administrators and the care team
  • Uphold company policies with integrity
  • Communicate honestly and professionally
  • Conserve resources and reduce waste and abuse of funds

Job Description

As part of the Care Team, the Care Coordinator is one of the first to interact with clients and caregivers, creating acaring culture with exceptional service. As a Care Coordinator, you are a key contributor to scheduling optimizationand utilization of hours which leads to client satisfaction and safety. Supports company initiatives by adopting newapproaches, practices, and processes to deliver standards of excellence and operational excellence on a consistent basis.The Care Coordinator will build strong relationships with caregivers to create a positive environment where caregiversfeel valued and supported. Essential Job Functions
  • Maintain quality service by following Outreach Standard Operating Procedures to onboard new clients timely andensure accuracy. This includes duties such as: accepts and coordinates referrals, client welcome calls, data entryinto EHR (electronic health record), eligibility checks, and caregiver matching
  • Communicate effectively with referral sources to correct any client registration or authorization issues
  • Maintain clients’ EHR (electronic health record) by entering new information timely and accurately
  • Address client concerns timely and with empathy
  • Supports other members of the Care Team to provide the best customer service possible and creating anenvironment of trust and collaboration within the team
  • Follow Outreach Standard Operating Procedures to recruit and onboard caregivers, acknowledging the importanceand impact of every detail. This includes duties such as: recruiting, interviewing, new hire paperwork, conductingreference checks, and coordinating orientation and training with Care Managers
  • Communicate with case managers, practitioners, and others as needed to facilitate client services and to ensureclient continuity of care
  • Assist in the direction and coordination of quality control in service delivery
  • Monitor and manage caregiver master schedule(s)
  • Assist in the overall flow of paperwork and ensure accuracy, completeness, and timeliness
  • Assist with incoming and outgoing calls with kindness and professionalism
  • Monitor proper usage of Electronic Visit Verification (EVV) Reason codes, enters client visits, and runs checks andbalances prior to submitting to payroll
  • Generate timesheets, ensures timely transmission to payroll, and reconciles billing
  • Compute, records, and submits initial billing, conducts all necessary research on outstanding billings, and re-submits outstanding billings timely
  • May reconcile A/R and A/P reports
  • May generate reports for use by Administrator and Care Team
  • Knows and follows Company policies and procedures with integrity
  • Conducts all communications in an honest and professional manner
  • Be a good steward of company resources, always striving to accomplish more with less and reduce fraud, waste, and abuse of funds

Preferred Skills, Education, Certification, License, and/or Experience
  • At least 2 years of relevant work experience
  • Bilingual (English and Spanish)
  • Experience with EHRs as well as tracking and monitoring Electronic Visit Verification (EVV)
  • Local knowledge about and connections to community health care and social welfare resources are desirable
