Job summary

Acentra Health seeks a Care Coordinator to join our growing team and help deliver high-quality care management services to plan enrollees.

You will perform outreach calls, conduct assessments, coordinate with case managers and health coaches, ensure service delivery in a compliant, patient-centered manner, and participate in ongoing quality improvements. This role is remote within Oregon with potential travel to the Tualatin office.

Qualifications

  • Associate’s degree in health-related field or medical assistant training.
  • 4+ years of experience in care coordination, preferably in field environments.
  • 2+ years in a clinical environment, preferably in field delivery.

Responsibilities

  • Inbound and outbound calls to enrollees and care team members.
  • Ensure comprehensive, compliant service delivery to enrollees.
  • Conduct general assessments for assignment to appropriate care staff.
  • Participate in interdisciplinary reviews for coordinated planning.
  • Support quality management activities and regulatory accreditations.
  • Assist leadership in budget-adherent service delivery and time/expense reporting.
  • Read, understand, and adhere to corporate HIPAA and security policies.

Acentra seeks a Care Coordinator to join our growing team.

Job Summary

Provides field care coordination outreach activities to support care management program delivery to plan Enrollees to support healthy lifestyle choices & to reduce short and long term effects of chronic illnesses.

Responsibilities
  • Responsible for inbound and outbound calls
  • Ensures the responsible delivery of comprehensive services to enrollees.
  • Conducts general assessments for supervisor/lead interpretation/evaluation and assignment to case manager / health coach based on results.
  • Participates in the interdisciplinary case reviews for collaborative assessment and coordination planning to ensure quality care.
  • Proactively engages in delivery of quality management program activities that are the direct responsibility of the Health Services team. Assists in the achievement and ongoing maintenance of accreditations for defined programs.
  • Assists Lead, Supervisor and/or Manager in ensuring achievement of contractual financial obligations, including service delivery in a cost effective and efficient manner and through support of budgetary adherence by reporting accurate and timely work hours and expenses incurred during course of position duties.
  • Read, understand, and adhere to all corporate policies including policies related to HIPAA and its Privacy and Security Rules.

*Work Schedule: Must have flexibility to work a shift between the hours of 7:0 AM to 7:00 PM Pacific Time, Monday through Friday. (Currently hiring into 10am-7pm coverage.)

*This role is remote within the State of Oregon and may require up to 5-10% travel to the Tualatin office depending on business needs.

Required Qualifications, Knowledge, and Experience
  • Associate’s degree in health-related field and/or medical assistant training
  • 4+ years of experience in care coordination (field environment preferred)
  • 2+ years of experience in clinical environment, preferably in field delivery
Preferred Qualifications
  • 1+ years of telephonic outreach or call center experience
  • Bilingual in Spanish strongly preferred
  • Public sector experience, e.g. Commercial / Medicare / Medicaid
  • Knowledge of principles, ethics, and precepts of care management service delivery
  • Knowledge of customer service principles
  • Knowledge of medical terminology
  • Knowledge of regulatory and accreditation standards
  • Strong written and verbal communication skills
  • Strong field and telephonic interviewing skills
  • Strong computer skills, e.g. care management applications, Internet/Web, Microsoft Office (Word, Excel, Access)
  • Strong prioritization and organizational skills
  • Ability to demonstrate purposeful and outcomes directed critical thinking within daily operational activities
  • Ability to receive verbal and written feedback in a professional manner and implement performance and productivity improvements as needed
  • Ability to interpret real-time and historical information to inform delivery on overall accountabilities

Experience in Lieu of Degree

For non-clinical roles, or when not required by the contract specifically, the Company acknowledges that practical, hands‑on experience can provide skills and competencies equivalent to formal education. As such, in cases where a Bachelor's degree may be required, the Company will accept a minimum of six (6) years of directly relevant professional experience in lieu of a degree. In instances where the candidate has an Associate's degree, the Company will accept a minimum of three (3) years of directly relevant professional experience in lieu of the Bachelor's degree.

Based on our compensation philosophy, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.

Pay Range

USD $23.50 - USD $28.89 /Hr.
