A leading company is looking for a Care Coordinator for their Integrated Care Clinic in Northfield, NJ. This full-time position involves customer service, appointment scheduling, and coordinating referrals for mental health and substance abuse services. The Care Coordinator provides customer service and access support functions including telephone and face to face screenings, scheduling appointments, consumer/ family orientation, education, and intake functions, as well as coordination of referrals for individuals seeking substance abuse and behavioral health services. Maintains an up-to-date referral list on a continual basis, triages referrals for services based on screening procedures, maintains an up to date wait list if applicable, and is available to staff and the community to provide information and coordination for consumers and efficient access to services.
Legacy Treatment Services' Integrated Care Clinic will serve residents of Atlantic County, New Jersey. The project will provide outpatient mental health and substance use counseling, person-centered treatment planning, case management services, peer support services, outpatient primary care screening and monitoring services, services for veterans, and crisis services.
Hours:
This is a 40 hour full time position, Monday – Friday, with some evenings required.
Education/Experience:
Bachelor’s degree in Psychology, Social Work, or related discipline. Experience working with the psychiatric population preferred.
