Position Overview

SCOPE OF ROLE:

The Care Coordinator provides services in the Clinical Home and in the community to HIV-positive clients living with complex medical conditions, severe mental illness, substance abuse, and long-term care needs, with a history or risk of over-utilizing medical and behavioral health services.

ESSENTIAL DUTIES & RESPONSIBILITIES:

  • Help find, engage, and enroll clients from the NYS DOH list of eligible Brooklyn and Manhattan Medicaid beneficiaries who have chronic illnesses and possible mental illness and/or substance abuse.
  • Ensure clients reliably attend medical appointments at outpatient clinics.
  • Ensure clients comply with prescribed regimens.
  • Write clear service plans and progress notes; maintain meticulous documentation of service visits.
  • Manage a caseload of clients, each to be visited at least once per month with at least one service provided during that visit.
  • Coordinate ancillary care to ensure continuous, comprehensive care.
  • Collaborate regularly with team members, primary care and specialty doctors, and all partners.
  • Initiate appropriate referrals to additional services including home care, and notify support staff to ensure authorization.
  • Assist clients in obtaining transportation access.
  • Research, evaluate, and recommend community resources to meet non-medical needs (e.g., support groups, community services).
  • Work with inpatient providers, discharge planners, and nurse partners to optimize inpatient resource use and facilitate timely discharges.
  • Participate in client care conferences for medical and peer review.
  • Collaborate with mental health and substance abuse providers to coordinate and integrate services.
  • Work with the ValueOptions Field Care Coordinator to link clients to illness management services.
  • Provide verbal and written education on illness management protocols.
  • Report serious incidents or sensitive situations immediately, completing incident reports per CIDP policy.

Locations available: Bronx

Bilingual English/Spanish is strongly preferred.

Qualifications

REQUIRED EDUCATION AND EXPERIENCE

- B.A. degree in a related field.

- 2 years’ experience providing direct services to mentally disabled clients or linking them to essential community services.

PREFERRED QUALIFICATIONS & SKILLS

  • Basic knowledge of mental illness and serious emotional disturbances.
  • Basic understanding of treatment, rehabilitation, and community support programs relevant to clients, families, and staff.
  • Bilingual English/Spanish.
