Job summary

High Plains Mental Health Center is seeking a Care Coordination RN in Hays, Kansas. The role focuses on guiding patients through transitions from hospital or treatment settings back into community care.

You will collaborate with hospitals, providers, and community partners to ensure safe, continuous, and well-coordinated care for individuals in our region.

Qualifications

• Current RN license with KS Board of Nursing.
• BSN preferred.
• Experience in psychiatric/mental health nursing or related settings.
• Experience in care coordination or case management.
• Strong interpersonal and collaboration skills.
• Ability to communicate treatment plans and transitions.

Responsibilities

• Help patients transition from inpatient psychiatric units, hospitals, and residential programs back into community care.
• Coordinate with providers, nurses, and community partners to keep treatment plans aligned and effective.
• Review medications and coordinate follow-up care to improve patient safety.
• Educate patients so they understand their care plan and feel confident moving forward.
• Build relationships across healthcare systems that strengthen mental health care in our region.

Qualifications

Current Registered Nurse (RN) licensure from the Kans s State Board of Nursing. Preferred- Bachelor Of Science In Nursing (BSN). Preferred: experience in psychiatric/mental health nursing, or physician office or outpatient practice. Preferred: experience in care coordination or case management. Strong interpersonal and collaboration skills when working with multidisciplinary teams and external providers. Ability To Communicate Effectively With Patients And Families About treatment plans and care transitions. Demonstrated proficiency with technology including computers, electronic records, and common office equipment. Commitment to continuous learning and quality improvement, particularly in mental health care and clinical processes.
