Those moments require someone who can ensure nothing falls through the cracks. At High Plains Mental Health Center, our Care Coordination RN will play a critical role in helping individuals successfully navigate these transitions. Youll work alongside hospitals, psychiatric facilities, providers, and community partners to ensure patients receive safe, continuous, and well-coordinated care. Current Registered Nurse (RN) licensure from the Kans s State Board of Nursing. Preferred- Bachelor Of Science In Nursing (BSN). Preferred: experience in psychiatric/mental health nursing, or physician office or outpatient practice. Preferred: experience in care coordination or case management. Strong interpersonal and collaboration skills when working with multidisciplinary teams and external providers. Ability To Communicate Effectively With Patients And Families About treatment plans and care transitions. Demonstrated proficiency with technology including computers, electronic records, and common office equipment. Commitment to continuous learning and quality improvement, particularly in mental health care and clinical processes.
