Behavioral Health Solutions in Kansas is seeking a Behavioral Health Care Coordinator to support long-term care and skilled nursing facilities. You will travel within your region to deliver onsite support, coordinate services, and engage with facility leaders, clinicians, and staff.

The role emphasizes collaboration, documentation, consent management, education, telehealth support, and ensuring PASRR II compliance, with a mix of office and field work in healthcare settings.

Qualifications

  • Must have high school diploma or GED; college preferred with focus on behavioral health/psychology.
  • 3–5 years in skilled nursing facilities as a Licensed Social Worker, Support Staff, or Clinical Care Coordinator.
  • Proficient with PC Windows, Internet, Outlook, Word and Excel; Athena EHR preferred.

Responsibilities

  • Travel to SNF facilities to provide onsite support within the assigned region.
  • Coordinate psychiatry, therapy, and neuropsychology services where available.
  • Assist facilities with referrals, consents, and education on BHS processes.
  • Attend meetings and coordinate physician attendance for facility meetings.
  • Ensure PASRR II compliance and schedule in-services and telehealth visits.

As a Behavioral Health Care Coordinator, you are an integral part of our team responsible for coordinating and providing essential services to patients in the long‑term care and skilled nursing facilities that we support. Our mission is to deliver exceptional behavioral health services to those in need, and this position plays a crucial role in cultivating positive relationships with the facilities in your assigned region. You will be actively involved in ensuring seamless coordination of services between our organization, facility staff, providers, and patients.

This job operates both in a skilled nursing facility and a typical office environment; noise levels vary from quiet to moderate, including ambient sounds common in a healthcare setting. Employees interact with and work near co‑workers, residents, and healthcare professionals on a frequent basis and must be able to remain stationary for long periods of time while providing care and attending to residents' needs. The position relies heavily on the ability to use and navigate electronic devices and software for patient care and documentation.

Responsibilities
  • Travel: Provide onsite support and services at all locations within your assigned region as required.
  • Customer Service: Engage and collaborate with facility staff, including Executive Directors, Directors of Nursing, case managers, social workers, staff developers, admissions personnel, surveyors, and physicians.
  • Service Coordination: Facilitate coordination of services such as Psychiatry, Therapy, and Neuropsychology (where available) to meet patient needs.
  • Referral Assistance: Assist facilities in processing referrals and identifying residents who could benefit from psychiatric and therapy services.
  • Consent Management: Coordinate completion of consents and provide assistance when needed.
  • Education: Educate facility staff on Behavioral Health Solutions processes, address questions, and resolve concerns.
  • Meetings and Collaboration: Attend behavior management meetings with facility staff and coordinate MD attendance for facility meetings such as GDR and QAPI.
  • Compliance: Ensure compliance with PASRR II requirements by working closely with BHS and the facility.
  • In‑Service Coordination: Schedule in‑services and educational sessions for facility staff.
  • Telehealth Support: Facilitate Telehealth visits with clinicians as needed.
  • Facility Onboarding: Collaborate with the Business Development team to support the facility onboarding process.
  • Other Duties: Perform additional tasks and assignments as specified by your supervisor.

  • Behavioral Health Expertise: Knowledge of behavioral/medical terminology, therapy, pharmacology, and related procedures.

  • Adaptability: Ability to multitask and thrive in a fast‑paced environment with evolving opportunities.

  • Relationship Building: Strong ability to build and maintain positive relationships with facilities, providers, and the supervisory team.
  • Communication Skills: Excellent written and verbal communication skills to interact effectively with various stakeholders.
  • Current COVID‑19 Vaccination: Adherence to safety protocols requiring vaccination against COVID‑19.
Requirements
  • Must be able to perform each essential duty satisfactorily.
  • Minimum of a High School Diploma or GED. Some college education is preferred, with a focus on behavioral health or psychology.
  • Experience: 3–5 years working in skilled nursing facilities as a Licensed Social Worker, Support Staff, or Clinical Care Coordinator.
  • Proficient in the use of a PC in a Windows environment, the Internet, and MS Office applications such as Outlook, Word, and Excel. Proficiency in EHR software systems (Athena) is also preferred.
Working Conditions
  • Must be able to remain in a stationary position more than 50% of the time during focus or administrative tasks.
  • Walk and move frequently inside the office and throughout the skilled nursing facility to access files, office machinery, facility contacts, and patients.
