Job summary

FirstLight® Home Care in Bucks County, PA is seeking a Care Coordinator who can manage client intakes, service oversight, care planning, scheduling, and supervision. This role interfaces with clients and caregivers across the county to ensure compassionate, professional in-home care.

The ideal candidate has home care experience, an associate degree, strong organizational and communication skills, a valid driver’s license, and lives within 25 minutes of Doylestown.

Qualifications

Responsibilities

• Manage day-to-day office and field operations for quality assurance of services.
• Recruit, hire and onboard caregivers.
• Answer phone and conduct intakes, take after-hours emergency calls, schedule issues, and client referrals/intakes.
• Perform initial and ongoing in-home evaluation, caregiver introductions, coordination, and supervision of client services.
• Assist with scheduling of shifts by matching caregiver qualifications and availability to clients’ needs.
• Handling caregiver call outs.
• Supervise and coach caregivers and conduct performance appraisals.
• Complete appropriate visit records on time and according to policy.

Job Summary:

This individual will be responsible for designated areas of organizational operations including, but not limited to client intakes and service oversight, service evaluations, creating care plans, scheduling and supervising care. The Care Coordinator will directly interface with clients & caregivers throughout Bucks & Montgomery Counties.

Care Coordinator responsibilities include:

• Manage day-to-day office and field operations to ensure quality assurance of services

The ideal candidate will have:

• Experience as a Care Coordinator or an extensive home care background.
• Associate degree or equivalent from a two-year college or technical school; and 2 years related health care/home care industry experience and/or training preferred; or equivalent combination of education and experience.
• Successful management experience preferred
• Proficient skills in Microsoft Office, Google Suite and scheduling software required. WellSky Preferred.
• Must possess and demonstrate excellent organizational, communication, interpersonal, and leadership skills as well as a positive and professional image
• Must possess and maintain a Valid Driver’s License and maintain adequate auto insurance
• Must live within 25 minutes of Doylestown, PA
• Ability to effectively present information and respond to questions from team members, clients, customers, and the general public
• Ability to evaluate situations and make complex decisions using critical thinking and reasoning.
