Position Summary
  • Ability to engage children/youth/adults in care.
  • Provide case coordination and care management for assigned clients.
  • Document services accurately in accordance with Medicaid and agency standards.
  • Participate in care planning and multidisciplinary meetings.
  • Collaborate with identified sources and service providers to complete timely referrals.
  • Maintain current knowledge of community resources that address the needs of individuals served.
  • Monitor client progress and reassess level of care needs.
  • Identify and resolve barriers to treatment engagement.Maintain regular communications with stakeholders to coordinate services.
  • Advocate for client access to appropriate services and supports.
  • Serve as a connector to services based upon identified diagnostic assessments.
  • Support continuity of care across systems (behavioral health, school, child welfare, medical).
  • Perform other duties as assigned.
What We Value In Our Next Care Coordinator
  • Bachelor's degree in social work, counseling, or related behavioral health field.
  • Experience working with children with behavioral health needs.
  • Valid driver's license, auto insurance and reliable transportation (if applicable).
