Job summary

Community Options seeks a Care Coordinator in its Behavioral Health Department to manage client caseloads, conduct intakes, assess needs, and develop care plans that promote independence by overcoming barriers. You will build strong client relationships, coordinate supports with other service providers, and facilitate client team meetings with timely follow-up to achieve goals.

Qualifications

  • An associate degree in a related field is required, OR 1 year of direct services experience.
  • Requires a valid driver’s license, acceptable driving record, reliable transportation, and proof of insurance.
  • Successfully pass a criminal background check, drug screening, and ND DHS pre-employment approval.
  • Care coordination training provided by the State of North Dakota after hire.
  • Understanding of human services and developmental disabilities.
  • Demonstrated ability to lead people and get results through others; team building.
  • Ability to plan over a 1-2 year time span.
  • Ability to analyze problems; develop solutions at functional and strategic levels.
  • Knowledge of policies and procedures; basic accounting skills.
  • Strong customer service orientation.
  • Commitment to mission and values.
