The role emphasizes collaboration with nursing staff, physicians, and administrators to optimize length of stay, throughput, and care transitions in a busy teaching environment.

Qualifications

• Bachelor's Degree in Nursing required; Masters in Healthcare preferred.
• Ability to develop strong working relationships with leaders and staff.
• Ability to educate, motivate, and build high-performing teams.
• Strong organizational skills in a fast-paced healthcare environment.
• Minimum 3 years of progressive healthcare management experience.

Responsibilities

• Ensure care coordination plans comply with policies, standards and budgets.
• Support quality improvement with KPI evaluation and outcome monitoring.
• Lead Care Coordination Model initiatives, SOPs, and escalation huddles.
• Direct all aspects to improve length of stay, throughput and patient placement.
