Providence Medical Foundation is hiring a Care Management support specialist who assists the Care Management team in coordinating patient care across the healthcare continuum. The role works closely with the healthcare team and performs clerical tasks identified during initial and ongoing assessments.

Qualifications

• Medical Terminology coursework or working knowledge.
• 1 year healthcare experience.
• Associate's Degree in a related field preferred.
• Medical Assistant Certification (CMA) upon hire preferred.
• Proficient in Microsoft Office (Word & Excel).
• Bilingual English/Spanish communication skills desired.

Responsabilités

• Support Care Management team in coordinating patient care across the healthcare continuum.
• Perform clerical tasks related to team functions and activities identified during assessments.
• Collaborate with the healthcare team during initial and ongoing member/patient management.
