Job summary

Comagine Health is seeking a Health Services Specialist to support clinical teams in coordinating and monitoring health care services. You will help manage referrals, schedule assessments, maintain records, perform data entry, and communicate with recipients, providers and care team members.

This role requires strong organizational skills, attention to detail, excellent communication, and the ability to coordinate multiple priorities in a fast-paced, team-oriented environment.

Qualifications

• High school diploma or GED required; post-secondary education a plus.
• 2 years of related work experience or customer service experience.
• 1 year of work experience in health care.
• Proficient with Microsoft Office Suite and familiar with database software.
• Strong organizational skills and attention to detail.
• Excellent oral and written communication skills.
• Excellent interpersonal and problem-solving abilities.
• Ability to coordinate multiple tasks in a team environment.
• Decisiveness in timely decision-making with internal/external customers.
• Ability to establish and maintain effective customer relationships.

Responsibilities

• Administrative & Program Coordination: manage timelines, track progress, and organize files.
• Scheduling & Service Coordination: arrange appointments and coordinate QMHP follow-ups.
• Service Monitoring: ensure services align with PCSPs and notify managers of gaps.
• Data & Systems Support: perform data entry in MMIS, Avatar, EDMS and other systems.
• Clinical Team Support: assist QMHPs, physicians, and team with scheduling and referrals.
