Job summary

Community Health Plan of Washington is seeking a Care Coordination support role to assist with member assessment, planning, and advocacy under supervision. The team focuses on equitable care management for members.

The position requires at least a bachelor’s degree in a health-related field and at least one year of care coordination experience, with strong Microsoft Office skills and knowledge of care management practices.

Qualifications

• Bachelor’s degree in social work, Practical Nursing Certificate, or equivalent health care field.
• Minimum of 1 year experience in care coordination or care management within health care settings (preferred).
• Experience with care management systems and planning techniques (preferred).
• Knowledge of Medicare and Medicaid regulations.

Responsibilities

• Coordinate care management activities under RN/LICSW Case Manager supervision.
• Communicate with providers to arrange follow-up appointments and obtain records.
• Assist members with transportation and access to community resources.
• Develop care plans and monitor progress with member-centric interventions.
• Document care management process in medical records per guidelines.
