• Treatment planning with clients.
• Anticipate Quality Assurance Data Needs.
• Plan and coordinate individualized health and social services.
• Ensure coordinated and efficient service delivery.
• Help clients with multiple problems needing assistance.
• Monitor and support clients throughout treatment.
• Advocate for clients with external resources.
• Assess client and family needs and coordinate services.

Education
