Job summary

CVS Health is seeking a Care Management Associate to coordinate medical services and support care plans. The role involves initial review of care tasks, triage, and facilitating efficient care delivery with nurses or medical directors.

Training spans 4 weeks plus 8 weeks with a preceptor, followed by 8-hour shifts at locations near New Albany, OH and surrounding areas. Qualifications include 2 years in customer service or related fields, proficiency in typing, and experience with MS Word,

Qualifications

• 2 years of experience in either customer service, telemarketing and/or sales.
• Proficiency in typing for keying in information and navigating in multiple systems.
• Experience with computers including knowledge of Microsoft Word, Outlook, and Excel.
• Requires long periods of sitting and telephone and computer work.

Responsibilities

• Care Team intake, screening and support the implementation of care plans.
• Identify and triage Care Team tasks and apply intervention assessment tools.
• Utilize systems to build, research and enter member information as needed.
• Assist in research and resolution of claims payment issues.
• Support administration of hospital care, case management and quality management processes in compliance with laws and standards.
