Job summary

CVS Health is seeking a Care Management professional in Ohio to coordinate care, support, and education for members, helping to drive outcomes through care management resources. You will assist in implementing projects and processes for Case Management in a dynamic health care environment.

Requirements include a Bachelor's in behavioral health or related field, willingness to travel up to 50% within the region, and ability to work beyond standard hours.

Qualifications

• Bachelor's degree in behavioral health, human services, health services, or public health.
• Willing to travel within the assigned region up to 50% of the time; mileage reimbursed.
• Willing to work beyond core business hours, Monday–Friday, 8am–5pm, as needed.

Responsibilities

• Coordinate care coordination, support, and education to facilitate healthcare outcomes.
• Help implement projects, programs, and processes for Case Management.
• Apply practical knowledge of Case Management to administer policies and plans.
• Engage with community resources and discharge planning when applicable.
