Job summary

SMA Healthcare in Florida seeks a Care Coordination Specialist to connect clients with post-acute services, monitor referrals, and work with community partners to ensure timely, appropriate care. The role emphasizes suicide risk assessment and ongoing safety support for clients.

You will engage clients discharged from acute care, coordinate follow-ups, and maintain contact throughout the 90-day care coordination period, ensuring compliant safety plans and effective linkages to services.

Responsibilities

• Engage clients admitted to the Central Receiving System to discuss care coordination benefits and obtain consent after discharge.
• Review Safety Plan/Wellness Toolkit upon discharge with client and support system.
• Arrange appointments within 7 days of discharge with verification within 1 business day post appointment.
• Provide follow-up calls within 24 hours and 72 hours after discharge.
• Care transfer check-ins over a 90-day period to refer/link mental health and substance abuse services.
• Track all client contacts during participation in the program.
• Assess suicide risk during care transfer check-ins and link to acute stabilization if needed.
• Performs other related tasks as assigned.

Job description

The Care Coordination Specialist assists clients who access SMA's Central Receiving System of Care by providing referrals and direct linkages for continued care after acute stabilization, and then monitoring those referrals and linkages to ensure client follow through. The Care Coordination Specialist also interacts with community partners and other behavioral healthcare organizations to help support clients. The goals of care coordination are to help to ensure that clients receive timely and appropriate services and to monitor the success of referrals/linkages. The Care Coordinator will also assess on an ongoing basis the client's level of suicidal risk, stressors, needs, and adequacy of safety measures/supports.

Essential JobFunctions:

Minimum Educationand/or Experience:

Possession of a High School Diploma orequivalent. Advanced degree with aconcentration in psychology, social work, mental health counseling, or humanservice degree preferred. Anunderstanding of basic principles of care coordination preferred.

Necessary SpecialRequirements:

Possession of a valid Florida driver's license,acceptable driving record, and proof of personal automobile insurance ifrequired to drive an SMA vehicle and/or use a personal vehicle for SMAbusiness. Complete State of Florida mandatory background screening prior tostart of employment. Complete SMArequired training during the first six (6) months of employment and updatedif required.
