Responsibilities

• Complete assignments using up-to-date subject knowledge to meet deadlines and follow procedures; collaborate to solve problems with limited guidance.
• Assist in initial case assessment by coordinating with patients and families, determining eligibility and benefits, and entering authorization data.
• Coordinate care plans, monitor authorizations, attend case management rounds with clinicians, and review diagnoses as needed.
• Partner in developing client-focused case management plans with goals aligned to patient and family needs under limited guidance.
• Stay updated on current research, policies, and procedures; attend seminars and workshops to maintain knowledge of community resources.
• Assist with patient disposition planning and obtain necessary authorizations.
