Job summary

The University of Iowa Health Care, Care Coordination Division, seeks a Social Worker III to provide direct social work services to patients and families, including assessment, treatment, interventions, and care planning to facilitate movement through the continuum of care. This role also supports interdisciplinary teams and may involve group therapy and program development.

Responsibilities include conducting assessments, identifying needs, coordinating resources, and contributing to care

Qualifications

• Master’s degree in social work required.
• Active Iowa social work license required.
• Excellent written and verbal communication skills.

Responsibilities

• Assess patients and families; develop and implement plans of care.
• Identify patients needing social work intervention through screening.
• Conduct social work assessments with patient/family involvement.
• Provide individual, group, and family therapy as appropriate.
• Collaborate with interdisciplinary team and provide psychosocial input.
• Serve as resource to team and advocate for social work role.
