Job summary

The role involves psychosocial assessments, crisis intervention under supervision (LMSW/LCSW), collaboration with healthcare teams, advocacy, and documenting mandated reports. MSW with LMSW/LCSW license and BLS are required.

Qualifications

• Master of Social Work from an accredited program.
• Licensed LMSW or LCSW, or in supervision.
• Basic Life Support certification required.

Responsibilities

• Assess patients using data from medical diagnoses, chart reviews, interviews, and care teams.
• Performs psychosocial assessments and relates findings to clinical staff.
• Coordinates care plans including resources, crisis intervention, prevention, education, patient advocacy, and support.
• Educates patients and families about diseases, treatments, social or financial issues.
• Monitors care progress and adjusts interventions as needed.
• Acts as liaison between patients/families and clinicians/other agencies.
• Advocates for patient concerns and escalates ethical issues as appropriate.
• Precepts LMSW/LCSW students when applicable.
