Job summary

OU Health is seeking a part-time Care Management Social Worker in Oklahoma City to coordinate care for adults and pediatrics within the OU Health system. Responsibilities include psychosocial assessments, care planning, crisis intervention, and patient education, with collaboration across the healthcare team.

Level 1–3 roles apply based on education, experience, and licensure. The position offers a total rewards package and rotating weekends. LMSW or LCSW with supervision is supported.

Qualifications

• Master of Social Work from an accredited institution required.
• LMSW or LCSW under supervision eligible; licensure requirements apply.
• Current Basic Life Support certification required.
• Healthcare experience preferred.

Responsibilities

• Assess patients' conditions using medical data, interviews, and care teams.
• Perform psychosocial assessments and document findings.
• Develop and monitor care plans including referrals and crisis support.
• Educate patients and families about diseases, treatments, and finances.
• Serve as liaison between patients, families, and clinical staff.
• Advocate for patient concerns and ethical considerations; report as required.
