Qualifications

• High school diploma or GED required.
• Completion of Medical Assistant Training, Patient Care Technician Training, or CNA Training.
• 2 years of health care experience.
• Knowledge of medical terminology.
• Bachelor's degree in Social Work preferred.

Responsibilities

• Verify insurance eligibility.
• Conduct outreach calls to coordinate care after discharge.
• Coordinate referrals to Care Management.
• Optimize patient/family experience in hospital and ambulatory care.
• Collaborate with providers to identify care coordination solutions.
• Provide member navigational services.
• Research patient concerns and provide resolutions.
• Document client data in the medical record.
• Coordinate multiple appointments using scheduling systems.
• Identify care gaps and coordinate actions to close gaps.
• Meet and accompany patients/families to initial appointments.
• Build relationships with physicians, payers, and healthcare teams.
• Communicate information to meet utilization/regulatory requirements.
• Educate internal healthcare team on care management concepts.
• Work independently to provide excellent patient care.
