Job summary

SEARHC’s Mt. Edgecumbe Medical Center in Sitka seeks a Manager of Care Coordination to lead a team of care coordinators, social workers, and discharge planning staff.

You will oversee inpatient care coordination, ensure timely discharges, and optimize resource use across MEMC and related facilities. The role requires a Bachelor’s degree, Alaska RN license, and at least 5 years in discharge planning with supervision experience.

Qualifications

• Bachelor’s degree required; advanced degree preferred.
• Current, unrestricted RN license in Alaska or another U.S. state.
• Direct hires must apply for Alaska RN license prior to start date.
• Agency staff must hold an active Alaska RN license.
• Licensed Vocational Nurse (LVN) in AK or another state may be considered.
• Clinical Competency required within 3 months of hire and every three years.
• Manager Competency required within 3 months of hire and every three years.

Responsibilities

• Provide team leadership and supervision while fostering a collaborative environment.
• Manage daily assignments and schedules to ensure balanced workload.
• Mentor new staff and provide ongoing training as needed.
• Build relationships with post-acute providers through community networking.
• Ensure quality of care by monitoring performance and implementing improvements.
• Collaborate with leadership to align departmental goals with the organization’s mission.
• Support case managers and social workers in resolving complex cases.
• Monitor and improve service delivery, efficiency, and patient outcomes.
