Job summary

Bluestone Physician Services is seeking a Care Coordination Data Entry Specialist to support operations by handling enrollment tasks, member outreach, and data entry across state systems. The role supports members across Minnesota, Wisconsin, and Florida, with day shift hours and full-time pay structure.

Ideal candidates have 3+ years in a related field, Associate degree preferred, and proficiency with Google Suite.

Qualifications

• Associate degree preferred or higher in Human Services or related field.
• 3+ years in related healthcare or administrative role.
• Google Suite proficiency required.

Responsibilities

• Mail out time sensitive documents to members.
• Conduct outreach calls to members or guardians.
• Work efficiently in the MnChoices system.
• Look up members in MNITs for eligibility of waivers and insurance information.
• Stay up to date with health plan changes and requirements.
• Participate in regular team and health plan meetings.
• Other duties as assigned by Operations Manager.
