Job summary

UF Health in Jacksonville seeks a Case Manager to participate in patient care planning and discharge planning within the treatment team. The CM will help ensure appropriate use of resources and promote safe, efficient throughput across the care continuum.

Ideal candidates have ASN or MSW with 1–2 years of discharge planning or case management experience in a medical setting. Strong communication and organizational skills, plus PC proficiency, are essential.

Qualifications

• Age of Patients Served: All Age Groups.
• Required Education: ASN or MSW or related field.
• New hire RNs must have a BSN within 3 years of hire or transfer.
• Preferred Education: BSN, MSW, Case Management Certification.
• Necessary Skills: Excellent verbal and written communication, excellent organizational skills, strong PC skills.
• Required Licensure: Florida RN licensure or MSW in lieu if applicable.
• Required Experience: 1–2 years discharge planning or case management in medical setting.
• Preferred Experience: Hospital discharge planning and case management.

Responsibilities

• Develop patient's plan of care and discharge plans.
• Facilitate throughput and conduct utilization review.
• Collaborate with patient, caregivers, attending physician, UR Coordinator and care team.
