Job summary

UF Health is seeking a Case Manager to join the treatment team. The CM participates in the development of the patient’s plan of care, facilitates throughput, conducts utilization review, promotes appropriate utilization of resources, and identifies needs to establish safe discharge plans.

Collaboration with the patient, caregivers, Attending Physician, Utilization Review Coordinator, and the treatment team is essential.

Qualifications

• Age of Patients Served: All Age Groups.
• Required Education: Associate Degree in Nursing, -OR- Masters in Social Work or related field.
• Preferred Education: BSN, MSW, Case Management Certification.
• Required Licensure/Certifications: Florida nursing license; Masters in Social Work accepted in lieu of RN.
• Required Experience: 1-2 years discharge planning, case management, managed care, or RN experience.
• Preferred Experience: discharge planning and case management in a hospital setting.
