Job summary

The role is largely clerical, but will involve moving around the hospital to deliver a Medicare document for signature and aiding RN Case Managers and Social Workers with their needs. Primary location: Downtown - Main Building.

Qualifications

• Under the direction of a licensed professional, provides clinical and clerical support to the Care Coordination Team.
• Coordinate care across the continuum (inpatient/outpatient/post-acute and community) to assure appropriate utilization of clinical and community resources.
• Duties include providing patient-centered care for coordinating care transitions of patients and families.
