Job summary

Luminis Health is seeking a Care Management Assistant to support patient transitions through coordination with care managers, social workers, and facilities. Responsibilities include processing referrals, arranging transportation, managing documentation, and facilitating discharge planning in a collaborative environment.

Qualifications

• Bachelor’s degree plus one year of related experience, or high school diploma or equivalent and three years related work experience.
• Experience in care coordination, Care management, disease management, hospital/acute care setting, or customer service preferred.
• Knowledge of State and Local regulations, community resources, discharge planning, insurance verification, financial counseling, and/or medical billing a plus.
• Ability to communicate effectively, required.

Responsibilities

• Interact with Care Management staff, insurance agencies, and post-acute care providers to coordinate patient care.
• Transmit required patient documentation and verify facility receipt.
• Arrange transportation and prepare transfer packets to support timely discharges.
• Communicate with post-acute facility liaisons and DME suppliers to resolve delays in placement or supplies.
• Deliver patient notification letters and obtain signatures for records.
