Job summary

Northwell Health is seeking a Discharge Planning Assistant for the Vassar Brothers Medical Center, a full-time role on a Monday–Friday schedule from 8:00am to 4:30pm. The position supports the Case Management team in meeting discharge planning and continuity of care needs for patients.

The role involves rounds participation, coordinating home care and durable medical equipment referrals, and verifying paperwork and payer details to ensure timely discharges and smooth care transitions.

Qualifications

• Discharge planning experience or exposure preferred.
• Clerical experience within health care settings is required.
• Ability to communicate with multiple care team members and external providers.

Responsibilities

• Attend unit rounds daily and communicate progress on discharge planning.
• Coordinate referrals to home care, infusion, wound care, and DME providers.
• Verify start/delivery dates and complete required paperwork (prior auth, medical necessity, etc.).
• Contact payers to verify eligibility and preferred providers.
• Relay precertification/preauthorization details to the case manager.
