Competenze

• 1–2 years hospital experience required.
• High School Diploma required.
• Basic Life Support (BLS) required.
• CCMA/CMA/RMA/NCMA credentials preferred.

Mansioni

• Under supervision of a licensed professional, provides clinical and clerical support to the Care Coordination Team.
• Coordinate care across inpatient/outpatient/post-acute and community to ensure appropriate utilization of resources.
• Assist RN Case Managers and Social Workers; move around the hospital to deliver Medicare documents and obtain signatures as needed.
