Care Continuum Partner - Medical Assistant

Join our team as a Care Continuum Partner - Medical Assistant at Lehigh Valley Health Network.

Position Overview: The Care Continuum Partner - Medical Assistant works regionally to support patients and practices, facilitating care both inside and outside the practice walls through advanced partnerships and virtual care technologies.

Key Responsibilities:

  • Support pre-visit planning and patient flow in collaboration with clinical teams.
  • Manage workflows for care gap closures, referrals, and medication authorizations.
  • Conduct patient outreach for care plan adherence via various platforms.
  • Facilitate virtual rooming and telehealth appointment support.
  • Provide patient education and instructions for tests and procedures.
  • Assist with annual well visit intake and manage prior authorizations and referrals.
  • Support practice empanelment and serve as a liaison for patient portal and medical record technology.

Minimum Qualifications:

  • Technical School or Specialized Diploma as a Medical Assistant.
  • At least 2 years of patient interaction experience and 3 years within a complex health system.
  • Attention to detail, excellent communication skills, and ability to handle multiple tasks.

Preferred Qualifications:

  • Associate’s Degree in Medical Assisting.
  • Experience with electronic medical records and scheduling software.
  • Knowledge of medical terminology and certification as a Clinical Medical Assistant (CCMA).

Physical Demands: Ability to lift 25 lbs, sit/stand frequently, keyboard use, and perform role-specific activities including kneeling, bending, and CPR.
