Job summary

Baton Rouge General seeks a dedicated care coordinator to partner with medical staff and the clinical team to manage outcomes for selected patient populations. The role ensures safe, cost-efficient progression of care throughout acute hospitalization and smooth transition back to the community.

Essential duties include facilitating access to appropriate care, coordinating with multiple departments, and educating patients and families on care plans and discharge goals.

Responsibilities

• Facilitates triple aim initiatives by ensuring access to appropriate care.
• Advocates for patients by assessing healthcare needs and appropriate levels of care.
• Coordinates care across hospital settings (ED, ICU, Burn unit, Women & Infant, med/surg/telemetry).
• Assesses patient situations by collecting information to develop progression-of-care management plans.
• Maintains documentation of planning, liaison, and coordination activities.
• Collaborates with post-acute coordinators to monitor discharge logistics and transition readiness.
• Speaks with patients and families to discuss goals of care and treatment options.
• Supervises nursing students and interns as appropriate.
