Job summary

Carease Health in NC seeks a compassionate Mental Health Provider to join our Collaborative Care team as a Care Companion/Social Worker. You will visit designated skilled nursing facilities, provide emotional support, and conduct resident assessments to identify psychosocial needs.

You will implement short-term therapy, document visit notes, and collaborate with psychiatric professionals, including a monthly meeting with the psychiatric NP, to ensure comprehensive care for residents.

Qualifications

• Bachelor’s/Master’s level education in social work, psychology, counseling or a related mental health field.
• Strong understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings.
• Excellent communication, documentation and relationship-building skills.

Responsibilities

• Visit designated facilities according to the schedule with your supervisor.
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs.
• Implement short-term, solution-focused therapy and supportive interventions.
• Document visit notes and maintain detailed records of interactions and interventions after each visit.
• Collaborate with psychiatric professionals, including participation in a monthly meeting with the psychiatric NP.
