Job summary

Carease Health in Memphis, TN is seeking a compassionate Care Companion/Social Worker to join our Collaborative Care team. You will provide emotional support, conduct brief mental health assessments, and deliver short-term therapy to residents in skilled nursing facilities, helping them feel heard and connected.

The role emphasizes collaborative care with psychiatric providers, regular supervision, and ongoing training.

Qualifications

• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings.
• Strong communication, documentation, and relationship-building skills.

Responsibilities

• Visit designated facilities in accordance with the schedule set with your supervisor.
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs.
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns.
• Document visit notes and maintain detailed records of interactions and interventions after each visit.
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.
