Job summary

Carease Health in Memphis, TN is seeking a compassionate Mental Health Provider to join our Collaborative Care team as a Care Companion/Social Worker. You will support residents in skilled nursing facilities with emotional care, brief therapy, and mental health assessments, contributing to their well-being and sense of connection.

You will visit facilities, perform assessments, deliver short-term interventions, and coordinate with psychiatric professionals.

Qualifications

• Associate degree minimum in social work or related field.
• 1 year of experience in mental health settings.

Responsibilities

Key Responsibilities

• Visit designated facilities in accordance with the schedule set with your supervisor.
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs.
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns.
• Document visit notes and maintain detailed records of interactions and interventions after each visit.
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.

Qualifications

• Associate, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field.
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings.
• Strong communication, documentation, and relationship-building skills.
• Education: Associate Degree
